We mail a census form to every household in early January. Please return it promptly.

New residents or residents who misplace the pre-filled census form we mailed may fill in the form below and return it to the Clerk's Office. It's fine to email a photo, but we need a real signature not a typed one.  You can also use the Town Hall drop boxes or mail to the Clerk's Office at 730 Mass Ave, Arlington, MA 02476.

Instructions for completing the Town Census

If all the information is correct, please sign and return the form.

If you are completing a form pre-printed with your household’s information and want to make corrections, please follow the instructions below.  If you are filling out a blank form, use one line for each person in your household. An explanation of the columns is also provided.

- Verify printed information. To correct, use the shaded line below. 
- To add household members, use blank lines. 
- To delete household members who moved, place an M in Column G and provide their address at the bottom of the form. To delete household members who are deceased, place a D in column G.
- List all family members (including those in military service, away at school, or living in a care facility) who declare Arlington their legal residence.

IMPORANT: This form cannot register a household member to vote or change a political party for a registered voter. To update a voter’s address, each voter must sign to confirm using the spaces at the bottom of the form. 

Column A – letter indicates the political party of registered voters
Colum D – write the occupation, not the place of employment
Column E – check the box if a household member works for a public safety agency. MGL Ch51.4(e) allows law enforcement and public safety personnel to keep their names off the street list.
Column F – if a household member is not a US Citizen, write the country of citizenship
Column H – check if the household member is a veteran
Column I – check if the household member is the spouse of a veteran

If two or more household members receive separate census forms, staple them together and provide a phone number. We will update the record to combine future census forms.

 

Massachusetts requires towns to compile a list of all residents every year. Returning the form promptly reduces follow-up mailings and costs. Registered voters who do not respond by June are moved to the inactive voter list and mailed a postcard they can use to confirm residence and restore their active voter status. You can find more information about inactive voter status on the Secretary of the Commonwealth’s website.
